Private Pay- Out of Network Provider (OON)

I am a private pay, out-of-network (OON) provider, meaning that I do not bill any insurance companies or EAPs directly. However, I am able to provide you with paperwork called a “super-bill”, which you may submit to your insurance company for reimbursement. Some insurance companies reimburse up to 40-80% of the billed cost. Please check with your insurance company to determine your out-of-network coverage. If it is helpful, you may provide the following information to your insurance company when inquiring about your out-of-network benefits:

Payment

Prior to your intake appointment, you will be asked to provide a credit card number to have on file. Your credit card will be billed at the end of your each session. You may also pay by cash or check at the end of each session.
